State Patrol Investigating Fatal Crash in Putnam County

Monroe Township – The Ohio State Highway Patrol’s Lima Post is investigating a fatal crash which occurred at approximately 3:55 P.M. on County Road E at the intersection of County Road 22, in Putnam County.

A 2004 Pontiac Grand Prix, driven by 58 year old Cindy R. Adams of Oakwood was stopped eastbound on County Road E, at the stop sign. A 2012 Chrysler Town & Country, driven by 16 year old Mikayla R. Mayes of Lexington, Kentucky was traveling southbound County Road 22. The Pontiac failed to yield the right-of-way, pulled into the intersection and was struck by the Chrysler.

Mrs. Adams was transported by air ambulance to St. Rita’s Hospital where she succumbed to her injuries. Ms. Mayes and her two passengers, Michelle Mayes, age 46 and McKenzy Mayes, age 13, were treated for minor injuries at the scene.

The Ohio State Highway Patrol was assisted on scene by the Putnam County Sheriff’sOffice, Putnam County Fire and EMS, Continental Fire and EMS, Mercy Health Life Flight and R & O Towing.

Mrs. Adams was not wearing a seatbelt at the time of the crash and injuries could have been minimized if the seatbelt had been in use. The Ohio State Highway Patrol reminds motorists to always buckle up, and to use caution at intersections allowing ample time for vehicles which have the right of way.
